Dear Parents/Guardians,
If your child is attending the Eighth Grade Formal on 6/11/2021, he/she must have a negative result from a Covid-19 PCR test; rapid tests are not acceptable. The test results cannot be any older than four days prior to the Formal.
We will administer the test to our students on 6/7/2021 at the MHS.
Students who are present in school that day will be called to the nurse for testing. Students who are virtual on 6/7/2021 can be tested between 1:30pm and 2:30 pm. They should enter via the rear/auditorium entrance to the building.
The attached form must be printed, fully completed, then brought with the student when he/she gets tested.
