First Day of Practices

Fall

All paperwork for middle & High School sports is due on or before July 29, 2022.     

 The first day of practice for high school athletics (Boys & Girls Cross Country, Boys & Girls soccer, Girls field hockey) will begin on
August 22, 2022.  Girls' tennis will begin on August 17, 2022.
Middle School Athletics will begin the first week of school.

         

Winter

Winter sports registration is now CLOSED.  11/28/22 Tryouts for Boys & Girls High School Basketball, cheerleading & indoor Track:  November 21 to November 23.  First official practice for High School & Middle School will begin November 28, 2022.             

 Spring



                REQUIRED ATHLETIC PAPERWORK - PLEASE READ CAREFULLY!!!

The following athletic paperwork must be fully completed and submitted before any athlete can participate in athletics. 

1. Online Registration - A fully completed online registration is required of all athletes.  Please click the link below to register. Note: Paper copies will not be accepted.

The documents below need to be downloaded, completed and returned to the South Amboy MHS main office. Please address them to Nurse Housman. Know that all documents can be found under the "Athletic Document" tab. Note: Only hard copies of these documents will be accepted.  

2. Physical
– A medical examination is required once per year, within 365 days prior of the first day of sports season.    

3. COVID-19 Health History Update Form - Signed and dated no more than 90 days prior to the first day of sport practice.

COVID-19 Physician Evaluation Form

This form needs to be completed by a physician before any athlete can participate or return to athletic activity if an athlete has answered "YES" to any questions on the Health History Update Form or COVID-19 Daily Pre-Screening Questionnaire.
